2025-2026 Sports Physicals
The staff of Nationwide Children's Hospital and Mercy Health Defiance will be on site to conduct our annual sports physicals on Wednesday, May 21st, during the school day. This will be for any student currently in grades 6th-11th. This is a great opportunity for our student athletes to get a physical for the 2025-26 school year. All students planning on participating in a sport next school year are required to have a physical on file in the athletic office before their first official practice. You will NOT be able to practice until the physical is on file in the athletic office.
This year all physicals are FREE of charge, it is highly recommended that you get your physical completed at school.
Forms and paperwork can be picked up in the Athletic Office. Or printed off from here.
The following highlighted areas and signatures need to be completed on the physical form BEFORE the physical can be given:
● History Form Completed and Signed, Disabilities Form Completed and Signed
● Name, Date of Birth, and Grade at top of Physical Examination Form
● Name, Date of Birth, and Grade at top of Medical Eligibility Form and Shared Emergency Information
● Student/Parent Signature and Date on Preparticipation Physical Evaluation, Health Concussion Info Sheet, and Sudden Cardiac Arrest Form
You will need the following on the day of your physical:
*Completed Physical Form with Signatures (please write next year’s grade on form)
It is highly recommended that the physicals be conducted here at the school.
